Player Forms Download

ALL PLAYERS PLEASE READ!!!
Whether you are a new or returning player, you must come to one of our open registration sessions with the following items:

• Your completed forms with all required parents signatures
• A photocopy of your medical insurance card.
• Your 2008 registration fee ($125.00 for grades 9-12, $75.00 for 7th & 8th grades)

All of the items listed above are required before you can participate in any practice or game.

Whether you are new to the club this year, or are a returning player, you must fill out new copies for the 2008 season, and provide proof of current medical insurance. There will be NO exceptions to this policy.

New Players: 
Complete the online registration form. After registering, you will be sent a confirmation email along with all required waiver forms.

Please make sure we have a valid email address and phone number when you register!

Returning Players:
If you played last season with OP Rugby, you are already registered, but you do need to download and print the USA Rugby Participant Waiver form, OP Rugby Medical Release form and the OP Rugby Transportation Release form.
To download forms:
Click on the form you wish to download. When the file download begins, select Save. If file download does not begin, or the file opens in your browser, you can save the file by right-clicking on the file name and selecting Save Target As.
